1. Consumer Rights & Health Records Requests
In compliance with Section 181.105 of the Texas Health and Safety Code, patients have the right to request access to their healthcare records maintained by this practice.
To request a copy of your records, please submit a written request directly to this office via email.
While processing requests, this practice follows all applicable state and federal laws. Fees may apply as permitted by law.
2. Notice to Clients: Information on Filing a Complaint
The Texas Behavioral Health Executive Council (BHEC) investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.
Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. Complaints must be submitted using the Council's official complaint form.
